I need Web script or program that will convert web form to pdf and email it to the person doing the input?

Does anyone know what script can do that? It puts the web form into a document like mail merge and convert to a pdf and email or download the pdf file to the person doing the input

You can easily do that using PHP. The form will post to a PHP page that will gather the variables and create a PDF document out of it.
